Job summary

Blue Summit Hospice and Palliative Care in Snellville/Athens area is seeking a PRN Social Worker to deliver psychosocial, emotional and bereavement services to hospice patients and families. You will conduct initial assessments, provide ongoing counseling, and collaborate with the IDG and community resources.

Requirements include an MSW or a BSW with supervision (CSWE-accredited programs preferred) and at least one year of healthcare experience; state requirements apply.

Qualifications

• MSW from CSWE-accredited program with one year of health care experience.
• BSW from CSWE-accredited program with one year of health care experience and MSW supervision.
• Bachelor's in psychology, sociology or related field with MSW supervision as required by state law.

Responsibilities

• Performs initial psychosocial, emotional, spiritual and bereavement assessments and contributes to IDG care planning.
• Provides ongoing counseling and crisis intervention to patients and families.
• Develops and maintains contact with community agencies to facilitate referrals.
• Documents psychosocial assessments and maintains patient records per policy.
• Acts as advocate for patient/family and participates in bereavement services.
