Supervision by an MSW is provided; strong interpersonal skills are essential to support patients and families facing end-of-life care within the IDG framework.

Qualifications

• MSW (CSWE-accredited) with one year health care experience or supervised equivalent.
• BSW with MSW supervision and MSW supervisor requirement.
• Related field degree (psychology/sociology) with MSW supervision and/state compliance.

Responsabilités

• Perform initial psychosocial, emotional, spiritual assessments and care planning.
• Provide ongoing counseling to patients and families and bereavement support.
• Document psychosocial findings and actions in patient records per policy.
• Coordinate with IDG and community agencies for referrals and support.
• Advocate for patient/family needs within the hospice team.
