Job summary

Blue Summit Hospice and Palliative Care in Roswell, GA is seeking a compassionate Hospice Social Worker to deliver varied psychosocial, emotional, and bereavement services to patients and families as an integral member of the IDG.

Qualifications include CSWE-accredited MSW with health care experience or CSWE-accredited BSW with supervision, or a related bachelor’s degree with MSW supervision; a minimum of 2 years’ experience is preferred.

Qualifications

• MSW from CSWE-accredited school and 1 year of health care experience or equivalent.
• BSW from CSWE-accredited school plus 1 year of health care experience with MSW supervision.
• Bachelor’s degree in psychology/sociology with 1 year of health care social work experience and MSW supervision.

Responsibilities

• Perform initial psychosocial, emotional, spiritual and bereavement assessments.
• Provide ongoing counseling and crisis intervention when indicated.
• Develop and implement goal-directed IDG care plans with physician approval.
• Act as patient/family advocate and coordinate referrals with community services.
• Document psychosocial assessments and interactions in patient records.
