Job summary

Housecall Providers is seeking a PRN Hospice Social Worker to deliver compassionate psychosocial support to patients and families in the Portland Metro area. The role focuses on evaluations, counseling, coordination of referrals, and bereavement services, with community-based practice 100% in the field.

Applicants must hold a Master’s in Social Work, an active Oregon LCSW/LMSW/CSWA license, at least one year of hospice experience, a valid driver's license, CPR certification, and be able to work

Qualifications

• Master’s degree in social work required.
• Current Oregon LCSW/LMSW/CSWA license required.
• Minimum one year hospice experience required.
• Valid driver’s license and automobile liability coverage or access to insured vehicle.
• CPR certification at hire or within 6 months.

Responsibilities

• Perform psychosocial/bereavement evaluations for patients and families.
• Participate in Interdisciplinary Group (IDG) to develop and evaluate care plans.
• Implement and evaluate plans of care for each patient/family.
• Assist with Advance Directives and POLST forms as needed.
• Provide counseling and education to patients and families.
• Coordinate referrals and bereavement services.
• Document in EMR with accuracy and compliance.
