Care at Home is seeking an experienced Social Worker (LCSW) to provide clinical social work services to patients and families in inpatient, ambulatory, and community settings. You will participate in multi-disciplinary case conferences, assess psychosocial needs, coordinate home care planning, and collaborate with care providers to support patient goals.

Minimum qualifications include a Master’s degree in Social Work and an LCSW license; NH licensure may be required by territory.

Competenze

• MSW and LCSW are required.
• Licensure in NH may be required depending on territory and operational need.
• One year of related experience; three years preferred.
