Job summary

Enhabit Home Health & Hospice is seeking a licensed social worker to deliver social services to patients requiring intermittent professional care, working under physician orders and the established plan of care. You will educate patients and families and collaborate with the health care team to support meaningful, home-based care.

Requirements include a master’s degree in social work from a CSWE-accredited program, valid state licensure, and at least one year in a health care setting.

Qualifications

• Must have a master’s degree in social work from a CSWE-accredited school.
• Must be certified or licensed in the state where practicing.
• Minimum of 1 year social work experience in a health care setting.
• Medicare home health or hospice experience preferred.

Responsibilities

• Provide social services to patients requiring intermittent professional services.
• Educate patients, families, and health care team according to the physician’s orders and plan of care.
