Luminos Hospice seeks a Hospice Social Worker to provide psychosocial assessments and counseling to patients and families under physician direction. You will collaborate with the Interdisciplinary Group/Team to coordinate comprehensive, compassionate end-of-life care tailored to each patient’s needs.

Qualifications

• MSW required from an accredited program.
• LCSW/LMSW or equivalent licensure required.
• Minimum 2 years of healthcare experience, hospice preferred.

Responsabilités

• Conduct psychosocial assessments evaluating emotional, social, and environmental factors.
• Provide counseling to patients and families coping with end-of-life issues.
• Coordinate care with the IDG/T to address all aspects of patient care.
• Assist families in understanding grief and loss and connecting with resources.
• Maintain documentation per hospice policies and regulatory requirements.
