Job summary

Queen City Hospice is seeking a Medical Social Worker to provide psychosocial support for hospice patients and families in Fayetteville, OH. You will assess needs, coordinate resources, offer counseling, and ensure services align with each patient’s plan of care.

The role requires a Bachelor’s in Social Work (Master’s preferred), current Ohio licensure, and at least 1 year of healthcare or hospice experience. A supportive, team-oriented culture awaits.

Qualifications

• Bachelor’s degree in Social Work is required (Master’s degree preferred).
• Current Ohio Social Work Licensure required (LSW, LISW, or LPSW-S).
• Minimum of 1 year of healthcare or hospice experience.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Develop and support individualized care plans.
• Provide counseling to patients, families, and staff.
• Maintain documentation per care plans.
• Assist with applications for SSI, Medicaid, Hospice benefits.
• Coordinate community resources and liaison with agencies.
