Job summary

UnitedHealth Group in Oregon is seeking a Social Worker to provide medical social services under physician direction and an interdisciplinary team, addressing social and emotional factors affecting health. Requirements include a CSWE-accredited Master’s in Social Work, an Oregon Social Worker license, and a valid driver's license;

Qualifications

• Master's Degree from a CSWE-accredited School of Social Work.
• Current driver's license and access to a dependable vehicle or public transportation.
• Licensed Social Worker in Oregon.

Responsibilities

• Assists patient, significant others, physician and team to understand social factors affecting health.
• Contributes to the development of an integrated Plan of Care for patients daily.
• Instructs team on community resources to support patients as needed.
