Job summary

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work, or equivalent.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• In Oregon, licensed as a Social Worker from the State Board of Social Work.

Responsibilities

• Assist patient, family, physician and health care team to understand social factors affecting health.
• Contribute to a comprehensive Plan of Care for patients on a daily basis.
• Instruct health care team on community resources available to assist patients.
