Interim HealthCare in Hudson, WI is seeking a Hospice Social Worker (MSW) to provide home-based social services to patients and their families facing end-of-life care. You will join a hospice team and help coordinate plans, resources, and counseling to support patients' goals at home.

The role requires an active Wisconsin MSW license, CPR certification, and at least 1 year of social work in home health or hospice settings. Excellent communication and compassionate care are essential.

Qualifikationen

• Minimum 1 year social work experience, ideally in home healthcare
