Job summary

Interim HealthCare of Western Wisconsin is seeking a Hospice Social Worker to visit patients in their homes and connect families with resources, counseling and support. The role emphasizes a 2-3 day workweek with mileage reimbursement and a 1:1 worker-to-patient ratio to maximize impact.

The position requires an MSW with Indiana licensure, at least one year in social work (preferably home healthcare), CPR, and knowledge of hospice regulations.

Qualifications

• Master’s degree in social work and active MSW license in Indiana.
• Minimum of 1 year of social work experience, ideally in home healthcare.
• CPR certification.
• Knowledge of state and federal home health or hospice regulations.

Responsibilities

• Provide home-based social services to patients and families dealing with end of life.
• Work as part of a hospice team that may include an RN, LPN, CNA, and/or Chaplain focused on the patient’s plan of care.
• Monitor a patient’s psychosocial condition and identify social and emotional needs.
• Conduct patient assessments, document progress and ensure patient is moving toward goals.
• Provide counseling, community resource planning, crisis intervention and advocacy.
• Consult with family and caregivers on patient’s plan of care and how to help them progress.
