The role covers home health, hospice, and palliative care settings, with emphasis on navigating complex care needs and collaborating with clinical teams. The ideal candidate holds an MSW (CSWE accredited) or higher, with relevant health-care experience, strong communication skills, and willingness to travel to off-site locations as needed.

Qualifications

• Master’s degree or above in Social Work (CSWE accredited).
• SWs hired before 2009 who have degrees in other fields are grandfathered in.
• SWs hired before 4/21/2019 who have BSWs are grandfathered in.
• Bachelor’s degree may be considered depending on geographic area (Note: Home Health requires MSW).
• Familiarity with EMR/EHR systems.

Responsibilities

• Performs and documents psychosocial assessment and plans.
• Empowers patient/families to negotiate with resource providers.
• Collaborates with other professionals to promote quality care and continuity of patient/family care.
• Participates in development of Plans of Care and case conferences.
• Addresses Bereavement, education, and outreach as applicable.
