Job summary

Inspire Hospice in the United States is seeking a compassionate Hospice Social Worker (MSW) to join our interdisciplinary care team. You will provide psychosocial support to patients and families navigating end-of-life care, including counseling, advance care planning, and connection to community resources.

This role requires travel across the service area and may include on-call rotations. The ideal candidate holds an MSW from an accredited program, demonstrates strong grief and loss

Qualifications

• Master's degree in Social Work (MSW) from an accredited program.
• Minimum of one year of experience in social work, counseling, or related healthcare field.
• Current automobile insurance and a valid driver's license; travel throughout service area.
• Knowledge of grief, loss, and family systems; excellent communication and counseling skills.

Responsibilities

• Provide counseling and psychosocial support to patients and families.
• Assist with advance care planning and goals of care discussions.
• Connect patients with community resources and coordinate referrals.
• Support grief and bereavement for families throughout and after hospice care.
• Complete documentation and participate in on-call rotation as required.
• Collaborate with interdisciplinary team to ensure comprehensive care.
