Job summary

BrightSpring Health Services, through its Adoration Health affiliate, seeks a compassionate MSW to join our hospice team in Indiana, PA. You will provide psychosocial support, counseling, and resource coordination to patients and families, ensuring dignity and comfort at the end of life.

This full-time role offers M-F 8AM–4:30PM with team collaboration, independent work ability, and opportunities to grow in grief counseling and family support services.

Qualifications

• MSW accredited by CSWE required.
• Bachelor's in social work CSWE-accredited acceptable.
• Minimum 1 year healthcare experience; hospice preferred.

Responsibilities

• Assess psychosocial needs of patients and families.
• Provide counseling, crisis intervention, and emotional support.
• Coordinate with care teams to support holistic patient care.
• Educate and assist with advance directives.
• Maintain accurate and timely clinical documentation.
• Provide referrals to community resources and support services.
• Support funeral, financial, and legal planning with families.

Our Social Workers provide emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect.

Qualifications

• A graduate of a master's program in social work (MSW) accredited by the Council on Social Work Education OR A graduate of a bachelor's program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in *418.114(b)(3).
• Minimum of one (1) years' experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
• Experience in a hospice care preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Current licensure according to Medicare guidelines to practice in state of operation.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
