Job summary

Blue Ridge Care is seeking a Hospice Social Worker to support patients, families, and caregivers through compassionate, psychosocial care in the home setting. You will collaborate with an interdisciplinary team to develop and implement individualized plans of care.

You will provide counseling, assess needs, coordinate resources, and document care in the EMR, while traveling throughout the service area to deliver high-quality hospice support.

Qualifications

• Master of Social Work required.
• At least one year of health-care/hospice experience preferred.
• Understanding of hospice philosophy and end-of-life care.
• Strong communication, documentation, and organizational skills.

Responsibilities

• Assess psychosocial status of patients and caregivers.
• Coordinate safety and environmental needs in home settings.
• Provide counseling and support to patients and families.
• Maintain accurate EMR-based records and assessments at bedside.
• Offer information on community resources and referrals.
• Collaborate with interdisciplinary team and participate in IDT meetings.
• Develop individualized care plans and contribute to quality improvement.
