Qualifications

• Bachelor’s or MSW degree in social work.
• Valid Pennsylvania Social Work license.
• Residence in Pennsylvania preferred.

Responsibilities

• Conduct psychosocial assessments and provide ongoing emotional support.
• Develop individualized care plans with the interdisciplinary team.
• Help patients and families access community resources and supportive services.
• Assist with advance directives and end-of-life planning.
• Provide crisis intervention and compassionate guidance.
• Offer bereavement support to families.
