Job summary

Affinity Hospice is seeking a Licensed Master Social Worker (LMSW) for full-time, on-site work in the Emory, GA area. The LMSW will assess psychosocial symptoms, develop objectives, provide psychoeducation, counseling, and casework interventions to support patients and families at end of life.

Key duties include collaborating with the Interdisciplinary Group, participating in on-call rotations, and guiding families through funeral planning, financial and healthcare decisions.

Qualifications

• Masters Degree in Social Work is required.
• Professional LMSW licensure in the state of employment is required.
• Clinical death, dying, and bereavement experience preferred.
• Knowledge of hospice state and federal regulations.

Responsibilities

• Provide psychosocial services to terminally ill patients and families.
• Assess psychosocial needs of patient and family.
• Participate in the Interdisciplinary Group and on-call rotation.
• Assist with funeral, financial, and health care planning as needed.
• Support bereavement care planning.
• Assist with funeral planning, financial, legal, and health care decisions.
