Job summary

Affinity Hospice is seeking a Licensed Master Social Worker (LMSW) to assess psychosocial symptoms in patients and families. You will develop psychosocial objectives, deliver counseling, and provide education to the Interdisciplinary Group to support holistic palliative care.

The role requires MSW, LMSW licensure, and clinical experience in death, dying, and bereavement, with a flexible schedule including some evenings and weekends. Full-time position with on-site hours in Alabama.

Qualifications

• Masters degree in Social Work required.
• Professional LMSW licensure in state of employment required.
• Clinical experience or training related to death, dying, and bereavement.
• Equivalency considered with related training and experience.

Responsibilities

• Provide psychosocial services to terminally ill patients and families.
• Assess patient and family psychosocial needs regularly.
• Assess pre-bereavement and anticipatory grief needs.
• Implement Plan of Care with social work goals and interventions.
• Attend weekly IDG meetings to identify risks and coordinate care.
• Participate in on-call rotation as assigned.
• Assist with funeral planning, finances, and healthcare decisions.
