Job summary

Hearth Hospice in Georgia seeks a Licensed Master Social Worker to deliver counseling and psychosocial support to hospice patients and families. You will assess psychosocial status, coordinate with the RN Case Manager and interdisciplinary team, and advocate for individualized care plans.

Schedule is Monday–Friday, 8am to 4:30pm, covering the Dade, Catoosa, Walker, Whitfield, Murray, Chattooga, Gordon, Floyd area.

Qualifications

• Master of Social Work required from an accredited school
• Licensed Social Worker authorized by the State Board
• Minimum 1 year experience in healthcare setting; hospice preferred

Responsibilities

• Assess psychosocial status and communicate with the RN Case Manager.
• Evaluate home situations, resources, and community support.
• Develop and implement individualized social work plans.
• Participate in interdisciplinary team meetings and family counseling.
• Provide empathetic counseling to patients and families.
