Job summary

Affinity Hospice is seeking a Licensed Master Social Worker (LMSW) to assess psychosocial symptoms in patients and families, develop objectives, and provide psychoeducation and counseling within the Interdisciplinary Group. The LMSW will deliver casework interventions to alleviate identified needs and participate in ongoing education and support for terminally ill patients.

The role requires a Master’s Degree in Social Work, SC LMSW licensure, and experience with death and bereavement.

Qualifications

• Master's Degree in Social Work required.
• Licensure as an LMSW in South Carolina.
• Clinical experience or formal training related to death, dying, and bereavement.
• Equivalent combination of specialized training, education and experience will be considered.
• Knowledge of hospice state and federal regulations.
• Ability to work a flexible schedule, including some evenings and weekends.
• Valid driver's license and auto liability insurance.

Responsibilities

• Provide a wide range of psychosocial services to terminally ill patients and their families.
• Regularly assess patient and family psychosocial needs.
• Regularly assess pre-bereavement/anticipatory grief needs.
• Implement Plan of Care to meet social service goals and provide counseling and referrals.
• Regularly re-evaluate patient and family psychosocial needs.
• Attend weekly interdisciplinary meetings to identify risks.
• Participate in on-call rotation with social work team as assigned.
• Assist family and patient in planning funeral, financial, legal and health care decisions.
• Perform other LMSW duties as assigned by management.
