Job summary

Optimal Care - Evergreen is seeking a licensed Hospice Medical Social Worker (LMSW/LCSW) to provide compassionate psychosocial care for patients and families in Grand Rapids, MI. You will conduct psychosocial assessments, crisis counseling, and support advance care planning as part of an interdisciplinary hospice team.

This role requires travel within the service area to homes, facilities, and community settings, with regular hours Monday–Friday and a strong commitment to culturally sensitive,

Qualifications

• Master's or doctoral degree from a CSWE-accredited school of social work.
• Current state licensure as LMSW or LCSW.
• Minimum 1 year of experience as a licensed Master's Social Worker.
• Willingness to travel to homes, facilities, and community settings.

Responsibilities

• Provide psychosocial care and counseling to patients and families.
• Perform comprehensive psychosocial assessments and crisis intervention.
• Support advance care planning and informed, values-based decisions.
• Collaborate with nurses, physicians, and hospice team to integrate needs into the care plan.
