Job summary

Ascend Hospice is hiring a Social Worker in Overland Park, KS to provide psychosocial care to patients and families facing terminal illness.

The requirements: Master’s in Social Work, LMSW, minimum 1 year of Bachelor’s SW experience (hospice preferred), willingness to travel, and strong verbal and written communication. Hours: M-F 8:30-5.

Qualifications

• Master's Degree in Social Work required.
• Minimum 1 year of Bachelor’s Social Work experience, hospice preferred.
• Licensed Social Worker (LMSW) certification required.
• Willingness to travel and strong verbal and written communication skills.

Responsibilities

• Assess emotional factors related to terminal illness.
• Assess environmental resources and obstacles to maintain safety.
• Provides emotional support, crisis intervention and individual/family counseling.
• Maintains contact with community agencies and services to facilitate referrals.
• Evaluates patient/family response to psychosocial interventions.
• Assesses cultural needs and family dynamics in end-of-life care.
• Performs comprehensive psychosocial assessments including risk of abuse/neglect.

Now Hiring – Social Worker – Overland Park, KS

Hours: M-F 8:30-5

The Social Worker will be responsible for but is not limited to:

• Assesses emotional factors related to terminal illness.
• Assesses environmental resources and obstacles to maintain safety
• Provides emotional support, short-term crisis intervention and individual and/or family counseling when indicated
• Maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
• Evaluates patient/family response to psychosocial interventions
• Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions
• Performs comprehensive initial psychosocial assessments Assesses the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect

Education:

Master's Degree in Social Work

Professional Experience:

• Minimum 1 year of Bachelor's Social Work experience, Hospice preferred
• Must have a Licensed Social Worker (LMSW) certificate
• Understanding of hospice philosophy related to families in crisis and end of life issues
• Ability to multitask & prioritize work load
• Must be willing to travel
• Strong verbal and written communication skills.
