Job summary

Phoenix Home Care in Hutchinson, KS is seeking a Hospice Social Worker to provide counseling and support to patients in the home, collaborating with the care team to coordinate resources and enhance quality of life.

The role follows a Monday–Friday schedule from 8:00 AM to 5:00 PM, offering a chance to make a meaningful impact for patients and families facing end‑of‑life challenges through compassionate, collaborative care.

Qualifications

• Master’s in Social Work (MSW) or LCSW required
• Hospice experience preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Professional, dependable, and organized

Responsibilities

• Provide counseling and support to patients and families in the home
• Assist with advance directives and community resources
• Participate in the hospice interdisciplinary team (IDG)
• Coordinate referrals and additional support services
• Maintain positive relationships with patients, families, staff, and referral partners

Their purpose is to provide emotional and practical support, with the goal of enhancing the well-being of those confronting end-of-life challenges. Hospice social workers collaborate closely with healthcare teams to organize treatments, services, and available resources, ultimately aiming to enhance a patient's overall quality of life.
