Job summary

Pathways Healthcare, a leading home health and hospice provider, seeks a licensed social worker to deliver psychosocial support to patients and families at the end of life. You will work with an interdisciplinary team to develop care plans, provide counseling, and coordinate community resources.

Requirements include a Master’s in Social Work, LICSW preferred (LCSW considered), CPR, and the ability to travel to patient homes.

Qualifications

• Current LICSW license preferred; LCSW considered as well.
• Master’s degree in Social Work from an accredited program.
• Two+ years of healthcare or hospice experience preferred.
• Current CPR certification with in-person skills verification.
• Valid driver’s license, reliable transportation, and required insurance.
• Strong interpersonal, communication, organizational, and problem-solving skills.
• Ability to travel to patient homes and community settings as needed.
• Experience with electronic medical records systems preferred (Homecare Homebase a plus).

Responsibilities

• Provide psychosocial assessments and emotional support to hospice patients and families.
• Build trusting relationships with patients, families, and caregivers with counseling and guidance.
• Coordinate with the interdisciplinary hospice team to support care plans.
• Participate in IDG meetings and daily briefings for coordinated care.
• Connect patients and families with community resources and support services.
• Maintain accurate documentation in the electronic medical record system.
• Communicate patient status updates and psychosocial needs to the clinical team.
• Support families through grief, adjustment, and end-of-life planning.
• Follow confidentiality, safety, and infection control protocols.
• Participate in case reviews, continuing education, and training.
• Participate in on-call responsibilities as assigned.
