The role focuses on psychosocial assessments, care planning, education, and collaboration within an interdisciplinary team to support patients and families through the hospice journey.

Qualifications

• Bachelor’s degree in social work required.
• MSW preferred; CSWE-accredited program.
• Active NC LCSWA/LCSW or SC LISW-CP licensure required/maintained.
• Proficiency with electronic medical records (EMR).

Responsibilities

• Conduct psychosocial assessments of patients, caregivers and families.
• Develop, implement, and evaluate plans of care.
• Incorporate culturally sensitive care and address spiritual beliefs.
• Document assessments, plans, and interventions per standards.
• Collaborate with the interdisciplinary group and VIA departments.
• Educate on hospice philosophy, advance care planning and resources.
• Lead family meetings and coordinate referrals to services.
