Job summary

VIA Health Partners is seeking an experienced hospice social worker to join our team in Uptown Charlotte, NC. You will assess patients and families, develop care plans, and coordinate with an interdisciplinary group to deliver compassionate end‑of‑life services.

The role requires an MSW with licensure as LCSW/LCSWA or LISW‑CP in NC/SC as applicable. Prior hospice/palliative care experience is preferred, with strong documentation and collaboration skills essential to success.

Qualifications

• MSW from an accredited program; CSWE accreditation preferred.
• Current active licensure as LCSW/LCSWA or LISW-CP in NC/SC where applicable.
• Clinical social work experience in hospice/palliative care preferred.
• Ability to document clinical work accurately and timely.

Responsibilities

• Provide psychosocial assessments for patients, families, and caregivers.
• Develop and implement plans of care, addressing counseling, education, and referral needs.
• Collaborate with interdisciplinary teams to coordinate care and resources.
• Document assessments and interventions per agency guidelines.
• Support transitions to grief services and coordinate referrals to community resources.
