Qualifications

• MSW from CSWE-accredited program with at least 1 year of healthcare experience.
• Bachelor's degree in social work with 1 year healthcare experience may be considered where MSW supervision is available.
• State licensure as required for the state of practice.

Responsibilities

• Explains hospice services and Medicare benefits to patients and families; obtains informed consent and election of benefits documents.
• Submits documentation to facilities after patient visits, timely per policy.
• Participates as a member of the IDT and contributes to plan of care.
• Provides psychosocial support and counseling to patient and family; coordinates with external counseling resources as needed.
• Identifies need for intervention by other IDT members and communicates needs clearly.
• Coordinates psycho-social care with IDT to ensure continuity and quality of care.
• Maintains updated care plans reflecting current problems, goals, and interventions.
• Supports advance planning including living wills/POA and memorial arrangements.
