Job summary

Trinity Health Mid-Atlantic is seeking a Part-time Social Worker II to cover MercyOne Genesis Hospice House and community hospice, with some driving involved. Shifts are Day Shift, 8:00am–4:30pm, Monday–Friday, PRN 8-hour daytime rotations.

The role provides psychosocial evaluation, counseling, casework, bereavement services, and outreach in collaboration with the hospice team. Licensure in IA/IL with a second license within 12 months is required, and a valid driver’s license is necessary.

Qualifications

• Master's Degree in Social Work.
• Licensure/registration required in Iowa or Illinois upon hire; second state license within 12 months.

Responsibilities

• Provide psychosocial evaluation and ongoing counseling to Hospice patients and families.
• Direct casework services and bereavement support.
• Community outreach and referral collaboration with physicians, staff and hospice leadership.
• Participate in quality monitoring and performance improvement activities.
