Qualifications

• Master's degree in Social Work.
• Field of Study: Social Work.
• Licensure in IA or IL upon hire; second license within 12 months.
• Valid driver’s license and Genesis minimum auto insurance.
• More than 2 years of social work experience.
• Ability to work with diverse individuals in stressful situations.

Responsibilities

• Provide psychosocial evaluation and ongoing counseling to hospice patients and families.
• Deliver direct casework and bereavement services.
• Conduct community outreach and referrals in coordination with care teams.
• Collaborate with physicians, staff, families and community agencies.
• Participate in quality monitoring and performance improvement activities.
