Job summary

Care Initiatives Hospice seeks a PRN Social Worker to provide compassionate psychosocial support across Council Bluffs, Avoca, Dunlap, and surrounding communities. You will assess needs, document care plans, counsel residents and families, and coordinate referrals with the care team to improve quality of life.

Applicants should hold an MSW with a current LMSW license, hospice experience preferred, and be willing to travel between multiple locations. A valid driver's license is required.

Qualifications

• Master of Social Work degree from an accredited program.
• Current LMSW license.
• Hospice or palliative care experience preferred.
• At least 2 years of verifiable social work experience preferred.
• Valid driver's license and ability to travel between locations.

Responsibilities

• Assess patients’ psychosocial needs and complete social histories.
• Document care plans, progress, and follow-up per regulations.
• Provide counseling and emotional support to residents and families.
• Coordinate referrals and liaise with community resources.
• Collaborate with the care team to improve quality of life.
