Trinity Health Mid-Atlantic is seeking a Part-time Social Worker II to cover MercyOne Genesis Hospice House and community hospice, with some driving involved. Shifts are Day Shift, 8:00am–4:30pm, Monday–Friday, PRN 8-hour daytime rotations.
The role provides psychosocial evaluation, counseling, casework, bereavement services, and outreach in collaboration with the hospice team. Licensure in IA/IL with a second license within 12 months is required, and a valid driver’s license is necessary.
Employment Type: Part time Shift: Day Shift Description: This Social Worker II position will cover the MercyOne Genesis Hospice House as well as community hospice which will require some driving. The PRN position is 8 hour day shifts Monday - Friday from 8:00am to 4:30pm.
To provide varied and ongoing social work services to Hospice patients and their families. This may include psychosocial evaluation, ongoing psychosocial counseling, direct casework services, bereavement services, community outreach and referral through collaboration with physicians, staff, families, community agencies and hospice leadership. Actively participate in quality monitoring and performance improvement activities.
Interaction is with a variety of people inside or outside the organization. Communications are often difficult or stressful in nature. Contact with others involves complex, detailed and often sensitive topics. The job requires a high degree of interpersonal skills to deal with a range of complicated problem situations. Interactions involve gaining the agreement of others.
