Job summary

VNA Care is seeking a Hospice Licensed Social Worker for Home Care, M-F, 8:00AM - 4:30PM, covering Canton/Needham territory. The role is 40 hours per week with compensation based on experience: $74,880 - $76,960.

The position focuses on counseling and casework for patients and families facing terminal illness, integrating physical, emotional, and spiritual needs and grief support. A CSWE-accredited social work education and MA licensure are required.

Qualifications

• Master’s Degree from a school of social work accredited by the Council on Social Work Education; or Bachelor’s Degree in Social Work with a minimum of 2 years’ experience in Hospice or PC; or Bachelor’s Degree in Psychology with a minimum of 3 years’ experience in Hospice or PC.
• Licensure Required: Licensed LCSW or LICSW to practice in Massachusetts.
• Experience Preferred: Minimum of one year experience in a health care setting

Responsibilities

• Provides counseling and casework services to patients/families that focuses on the management of the psychosocial status of the patient/family that are experiencing terminal illness.
• Counseling is rendered within the context of the physical, emotional, and spiritual needs of patients and their families during the last phase of life including grief recovery process.
• Supports patients and families in end-of-life care and related services.
