Qualifications

• Master’s degree in Social Work required and active license to practice in Kansas.
• CPR certification required.
• Strong interpersonal, communication and empathetic skills essential.

Responsibilities

• Provide home-based social services to patients and families in the home health setting.
• Assess psychosocial needs and monitor patient progress toward goals.
• Offer counseling and connect patients with community resources and support services.
• Collaborate with a home health team to implement the plan of care.
