Hospice of the Midwest in Forest Lake, MN is seeking a compassionate Social Worker to join our team. This role provides emotional support, counseling, and resource coordination to patients facing terminal illness in a home-based care environment. Schedule is Mon-Fri, 8:00 AM - 5:00 PM, with a focus on dignity and holistic care.

You will assess needs, educate families, and coordinate with community resources while maintaining thorough documentation and ensuring quality patient support.

Qualifications

• MSW (or CSWE-accredited bachelor’s in social work) required or eligible.
• Minimum of 1 year experience in healthcare; hospice experience preferred.
• Current license or good standing to practice social work in the state of operation.

Responsibilities

• Assess psychosocial needs of patients and families.
• Provide counseling, crisis intervention, and emotional support.
• Educate and assist with advance directives and community referrals.
• Maintain accurate clinical documentation and liaison with care team.
