Suite Living Hospice is seeking a dedicated Hospice Social Worker & Bereavement Coordinator to join our interdisciplinary team. This full-time role will Alternate between social work and bereavement coordination, serving patients and families in the South Metro Minneapolis–St. Paul area.

The ideal candidate holds an MSW from a CSWE-accredited program, Minnesota licensure, and experience in hospice or related fields. Strong counseling and coordination skills are essential.

Qualifications

• Master's degree in Social Work (MSW) from a CSWE-accredited program.
• Current social work licensure in Minnesota is required.
• Experience in hospice, palliative care, home health, or senior care is preferred.

Responsibilities

• Perform psychosocial assessments of patients and families.Provide counseling and crisis intervention.
• Develop care plans and coordinate social services across the hospice team.
• Deliver bereavement support and resources to families before and after death.
• Maintain accurate clinical documentation per regulatory requirements.
• Collaborate with interdisciplinary team and community resources.
