Emmanuel Hospice in Grand Rapids, MI is seeking a full-time Social Worker to provide holistic care for patients and families across a service area around Grand Rapids. The role includes assessing needs, developing and monitoring the Plan of Care, and collaborating with an Interdisciplinary Team to deliver compassionate care.

The position offers on-call coverage nights, weekends, and holidays, mileage reimbursement, and comprehensive benefits starting Day One.

Qualifications

• Current LMSW or LLMSW degree required.
• Flexible in adapting to nights, weekends and holiday on-call responsibilities.
• Mileage reimbursement at the federal rate.

Responsibilities

• Providing holistic care to the dying and their loved ones in a service area around Grand Rapids, MI.
• Assessing physical, psychological, social and spiritual needs of terminally-ill patients.
• Assisting patient and family with the Plan of Care to meet needs and monitor goals, with proactive interventions.
• Collaborating with an Interdisciplinary Team to plan, implement, and evaluate medical care.
