Job summary

Buckeye Hospice and Palliative Care Services seeks a qualified Medical Social Worker to provide comprehensive social work services to patients and families as part of the hospice care team. The role includes psychosocial assessments, care planning, and coordination with the Interdisciplinary Group to ensure holistic patient care.

Responsibilities include maintaining detailed clinical records, monitoring responses to interventions, and supporting discharge planning.

Qualifications

• Must engage in ongoing professional development through training and conferences.
• Must be licensed social worker in OHIO.
• Clear written and verbal communication required.

Responsibilities

• Provide Medical Social Services to patient and family per Plan of Care.
• Perform psychosocial assessment within 5 days of admission.
• Attend IDG meetings and contribute to patient care planning.
• Record and report changes in emotional and social factors.
• Maintain and submit clinical records (initial evaluation, care plan, daily notes).
• Evaluate responses to social work interventions and adjust as needed.
• Coordinate visits with Case Managers weekly for care coordination.
• Participate in IDG and QAPI meetings and activities.
• Support IDG if patient/family declines services.
• Assist in discharge planning as needed.
• Assess coping with dying for patient and family.
• Act as consultant on social, emotional, environmental factors.
• Help patients utilize family and community resources.
