Job summary

Buckeye Hospice and Palliative Care Services is seeking a Medical Social Worker to provide professional social work services to patients and families in the hospice setting. A psychosocial assessment is required, to be completed within 5 days of admission, with ongoing collaboration in the IDG to develop and adjust the care plan.

• Must be licensed to practice social work in Ohio.
• MSW preferred; BSW with supervision option acceptable.

Responsibilities

• Provide Medical Social Services to patients and families per the Plan of Care.
• Conduct psychosocial assessments within 5 days of admission.
• Attend IDG meetings and contribute to the care plan.
• Maintain and submit medical social work records and notes.
• Evaluate patient and family response to interventions.
• Coordinate visits with Case Managers on a weekly basis.
• Participate in IDG and QAPI meetings.
• Support IDG if patient/family decline services.
• Assist in discharge planning and caregiver support.
• Act as consultant on social, emotional, environmental factors.
• Help patients access community resources.
