ProHealth Home Health and Hospice in Columbus, GA seeks a Hospice Social Worker (PRN/Part-Time) to deliver psychosocial assessments, counseling, bereavement services and community education. The role supports an IDG team and involves coordination with families and providers.

The ideal candidate holds an MSW (CSWE-accredited) with healthcare experience or a CSWE-accredited BSW with MSW supervision, and a background in psychology, sociology, or related fields.

Competenze

• MSW from a CSWE-accredited program with at least one year of health care experience.
• BSW from CSWE-accredited program with supervision by an MSW and one year of health care experience.
• BSW or related degree with supervised experience in health care per state requirements.

Mansioni

• Perform psychosocial, emotional, spiritual and bereavement assessments for hospice patients and families.
• Provide ongoing counseling, crisis intervention, and case management under physician direction.
• Develop and update care plans, coordinate with IDG and community resources.
• Document visits and referrals accurately in patient records.
