Job summary

Aurora Health Care's hospice program seeks a social worker to provide initial psychosocial evaluations, ongoing counseling, bereavement services, and community referrals for terminally ill patients and families in the Green Bay area.

You will assess needs, develop and implement social work care plans with the hospice team, facilitate end-of-life conversations, and participate in QA/CQI processes to improve care quality.

Qualifications

• Master's Degree in Social Work from an accredited school of social work.
• One year experience in a medical clinical setting or hospice/palliative care preferred.

Responsibilities

• Provide initial psychosocial evaluations, ongoing counseling, bereavement services, and referrals for terminally ill patients and families.
• Assess psychosocial needs and develop a social work care plan with the hospice team.
• Evaluate hospice level of care with the interdisciplinary group and ensure smooth transitions between GIP, Routine, and Continuous Care.
• Facilitate end-of-life planning conversations including goals of care, funeral arrangements, caregiving, guardianship and legacy work.
• Participate in QA/CQI processes for Advocate Hospice.
• Support patients and families across Manitowoc and Green Bay territories.
