Blue Ridge Care is seeking a Hospice Social Worker to support patients, families, and caregivers with emotional and psychosocial needs in end-of-life care. The role collaborates with an interdisciplinary team to develop and support individualized care plans and to provide counseling, assessments, and resource coordination.

The ideal candidate holds a Master of Social Work, has at least one year in healthcare (hospice preferred), and demonstrates strong communication, documentation, and

• Master of Social Work required.
• At least one year of experience in a health-care setting; hospice experience preferred.
• Understanding of hospice philosophy and end-of-life issues.
• Strong communication, documentation, and organizational skills.
• Ability to travel throughout the Blue Ridge Care service area in Loudoun County.

Responsabilidades

• Assess psychosocial status of patients and caregivers.
• Assess home safety and environment; plan alternative care if needed.
• Conduct social evaluations and implement coping interventions.
• Provide supportive counseling to patients and families.
• Maintain accurate clinical records in the EMR.
• Offer information and referrals to community resources.
• Liaise with hospice team and community partners.
• Develop and access new community resources.
• Participate in IDT meetings and care planning.
• Engage in quality improvement activities.
