Job summary

Blue Ridge Care is seeking a Hospice Social Worker to support patients, families, and caregivers with compassionate, evidence-based psychosocial care. You will assess needs, coordinate community resources, provide counseling, and ensure safety and quality of life for patients in home settings.

Qualifications

• Master of Social Work required.
• At least one year of health-care or hospice experience preferred.
• Ability to travel within Loudoun County, VA.

Responsibilities

• Assess psychosocial status of patients and caregivers in the context of terminal illness.
• Provide counseling to patients and families related to stress, illness progression, and end-of-life issues.
• Maintain accurate clinical records and EMR documentation at the bedside.
• Coordinate information and referrals to community resources and services.
• Serve as liaison between families, hospice team and community partners.
• Develop and implement social interventions to support stability and coping.
• Collaborate in interdisciplinary team meetings and care planning.
• Participate in quality improvement activities.
• Support networking and resource development with public and private agencies.
