Job summary

Hospice of the North Coast is seeking a Social Worker to join our intimate team in California, under physician guidance, performing psychosocial assessments and counseling for patients and families facing end-of-life transitions.

The MSW will collaborate with the Interdisciplinary Team, applying cultural sensitivity and upholding our hospice philosophy. A Master's in Social Work and licensure or eligibility (ACSW/LCSW preferred) are required or preferred.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• ACSW or LCSW preferred but not required.
• Clinical experience relevant to psychosocial needs of patients and families.

Responsibilities

• Conduct psychosocial assessments and evaluations.
• Provide emotional counseling to patients and families.
• Collaborate with the Interdisciplinary Team on care plans.
