Job summary

Snowline Health’s hospice team seeks a compassionate Medical Social Worker (MSW) to provide patient- and family-centered support in our Rancho Cordova service area. You will collaborate with the interdisciplinary team to honor goals, ensure dignity at end of life, and connect families with resources.

The role requires an MSW with strong counseling and communication skills; clinical geriatric experience is preferred, and occasional evenings/weekends may be needed.

Qualifications

• MSW degree from a CSWE-accredited program.
• Licensed MSW preferred, LCSW preferred.
• Minimum 1 year of geriatric clinical experience; hospice experience preferred.

Responsibilities

• Provide counseling and emotional support to patients and families.
• Coordinate care with the interdisciplinary team to enhance comfort and quality of life.
• Navigate community resources and referrals; ensure timely documentation.
• Maintain productivity and meet organizational expectations; participate in team meetings and trainings.
• Respect diverse beliefs and values; adapt to patient and family needs.
