Cabrillo Hospice is seeking a compassionate Social Worker to join our interdisciplinary team and provide emotional support, education, and ongoing care to patients and families facing end-of-life transitions.

Qualifications

• MSW required; hospice experience preferred.
• Geriatrics or end-of-life care background a plus.
• Strong communication and compassionate patient-centered approach.
• Ability to work independently within a multidisciplinary team.

Responsibilities

• Conduct Comprehensive Psychosocial Assessments for patients and families, addressing emotional, social, and environmental needs.
• Provide Emotional Support & Counseling to patients and families navigating serious illness and grief.
• Collaborate with the Interdisciplinary Team to develop and support individualized plans of care.
• Connect Families to Community Resources and support services.
• Participate in and support hospice bereavement services for families.
• Maintain accurate documentation and ensure services align with hospice regulations and standards.
