Job summary

VITAS Healthcare is seeking a hospice social worker to provide emotional and logistical support to patients and families during an important final journey. You will ensure affairs are in order, assist with finances and funeral arrangements, and help families navigate grief with compassionate guidance.

Applicants should hold a Master of Social Work, have at least one year of healthcare social work experience, and demonstrate empathy and strong listening and communication skills.

Qualifications

• Masters of Social Work required from a school accredited by the CSWE.
• At least one year social work experience in a healthcare setting.
• Experience in providing counseling and support to patients and families.
• Empathy and compassion in dealing with seriously ill patients and their families.

Responsibilities

• Provide emotional and logistical support to patients and families during end-of-life care.
• Help patients organize finances, funeral arrangements, and personal affairs to honor their wishes.
• Offer bereavement support and grief counseling to families.
• Educate patients and families on emotional responses to illness progression.
• Provide counseling and crisis intervention services as needed.
