Job summary

VITAS Healthcare is seeking a hospice social worker to provide emotional and logistical support to patients and families during end-of-life care. You will help ensure medical and personal affairs are in order, respect patient wishes, and assist with funeral planning and financial arrangements.

You will collaborate with RNs, physicians, chaplains and other team members, travel as required, and provide compassionate counseling and crisis intervention to patients and families facing terminal

Qualifications

• Masters of Social Work required from a school accredited by the CSWE or equivalent.
• At least one year social work experience in a health care setting.
• Experience with the terminally ill and their families preferred.
• Experience in providing counseling and support to patients and families.
• Empathy and compassion in dealing with seriously ill patients and families.
• Licensure equivalence may be considered where allowed by state regulations.

Responsibilities

• Provide emotional and logistical support to patients and their loved ones.
• Help ensure end-of-life planning, including finances and funeral arrangements.
• Assist families in navigating the grieving process and offer bereavement support.
• Collaborate with an interdisciplinary team to deliver 360-degree patient care.
• Advocate for patient welfare and maintain patient-centered care.
