Job summary

VITAS Healthcare is seeking a hospice social worker to provide emotional and logistical support to patients and families at end of life. You will help with finances, funeral arrangements, and bereavement, coordinating with an interdisciplinary team to deliver compassionate, patient-centered care at home or in facilities.

You will travel to patient homes as needed and participate in weekly team meetings to ensure comprehensive support and continuity of care.

Qualifications

• Master of Social Work required from an accredited program.
• At least one year of social work experience in a health care setting.
• Experience with terminally ill patients and families preferred.
• Ability to provide counseling and bereavement support.
• Demonstrates empathy and compassion when working with seriously ill patients and families.
• Licensure or equivalent experience as allowed by state regulations.

Responsibilities

• Provide emotional and logistical support to patients and loved ones during end-of-life care.
• Help patients reach end-of-life goals, including finances and funeral arrangements when appropriate.
• Support families through the grieving process and bereavement needs.
• Collaborate with the interdisciplinary team and visit patients at home or in facilities.
• Contribute to planning and coordinating 360-degree care with RNs, physicians, chaplains, and other team members.
