Gentiva is seeking a Hospice Social Worker (MSW or BSW) to join our nationwide team. You will serve as the psychosocial expert on the interdisciplinary care team, conducting assessments, counseling, and resource planning for patients and families facing serious illness and end-of-life care.

The role requires CSWE-accredited MSW/BSW, current state licensure, and at least 1 year of hospice or healthcare social work experience. Strong communication, empathy, and cultural sensitivity are essential.

Qualifications

• MSW or BSW from CSWE-accredited program.
• Current state licensure as required.
• Minimum 1 year of healthcare or hospice social work experience.
• Knowledge of psychosocial dynamics of illness, grief, end-of-life care.
• Strong communication skills, boundaries, and cultural sensitivity.

Responsibilities

• Serve as the psychosocial support expert on the interdisciplinary care team.
• Provide assessments and care planning based on emotional, psychological, environmental, and cultural needs.
• Deliver counseling, crisis intervention, and practical resource support.
• Work with patients, families, physicians, nurses, chaplains, and other team members.
• Educate families on hospice services, advance care planning, grief, and coping strategies.
• Assist with long-term planning, community referrals, and resource navigation.
• Monitor psychosocial changes and update care plans as needed.
• Uphold ethical standards and professional judgment in end-of-life care.
